Role overviewAt Aveo, we are seeking an experienced Nurse to join our team as a Clinical Case Manager at Aveo Freedom Aged Care facility, located in Tweed Heads. In this role, you will be committed to ensuring high quality service and care are provided to all of our residents. You will work closely with our wonderful team and liaise with residents, families and external health professionals on a regular basis.About The Role: Part time position (Monday to Wednesday)Monitor the work practices of Personal Carers and provide direction and guidance as required to ensure the provision of care to the required standard.Assist in the assessment of potential new residents and respite clients;Determine eligibility for externally funded care services;Establish comprehensive care plans aligned to assessed care needs;Provide leadership to and clinical governance of RNs, EENs, Personal Carers and other non-clinical staff;Assist the Care Manager in managing local HR issues as required or delegated;Ensure all residents' preferences and lifestyle wishes are respected and promoted;Provide a culture of lifestyle activities and wellness that improve the outcomes of residents' lives. To be successful in this role you will have: Minimum 3 year's nursing experience preferably in the aged care industryCurrent AHPRA registration Div 1 requiredCurrent Police Check certificateCurrent CPR certificateClinical skills and good mentoring of staff skillsAble to produce good care plans and clinical assessments The ideal candidate will possess: Competent with 'Packages' and networking with providersExcellent communication skills and working collaboratively as part of a teamDemonstrated problem solving skillsDemonstrated ability to deliver high quality clinical care with compassionIntermediate level of computer competence the Microsoft Suite and on-line network applications About Aveo:Aveo's vision is to be Australia's leading and most innovative seniors living provider.
