We are committed to our vision of 'better health services for the people of western Sydney and beyond' and to the delivery of health care or related services in line with our core values of collaboration, openness, respect and empowerment.<br /> <br /> <strong>What you'll be doing</strong> <div style="font-size:10pt">The Occupational Therapist will be a member of the multidisciplinary Drug Health Assertive Community Management (ACM) team which provides psychosocial assessments, interventions and support to consumers with a range of comorbid physical health problems and multiple psychosocial issues including accommodation and housing, social isolation, and vocational. This position will provide assertive follow-up for consumers including home visits. <br /> <br /> This position is responsible for the provision of high quality client centred, evidence based Occupational Therapy services to clients of WSLHD under the direction of the ACM Team Leader and the Allied Health Manager, Drug Health WSLHD. The Occupational Therapist will be an integral member of the ACM team assisting consumers with managing basic aspects of daily life and increasing independence and self-confidence.
