Live in paradise, where the rainforest meets the reef.About the Role The Lead Nurse Navigator is a Registered Nurse with advanced clinical knowledge who is accountable at an advanced practice level for the coordination of complex patient care, who supports the Nurse/Midwife Navigators in providing individual assistance and care coordination to patients with a chronic/complex disease profile and their families and care givers to help overcome healthcare system barriers and improve patient experiences.
