The role includes; Focused assessment and management of patients using advanced knowledge and skills which may include but not limited to ordering diagnostic investigations, prescribing medicines and direct referrals to other health practitioners. Is grounded in the nursing professions values, knowledge, theories and practice and provides innovative and flexible health care that compliments other health providers. The scope of practice is determined by the context within which the Nurse Practitioner is authorised the practice. Provides leadership that facilitates the ongoing development of clinical practice utilising research in the provision and development of educational programs, clinical services planning and development of clinical practice standards. Is regarded as a senior nursing clinician and resource person for the service and works closely with clinicians and management in the provision of care.Manages and cares for patients, families and significant others through utilising models of care that are appropriate, timely and of a high quality which are governed by authorised guidelines/polices. The After Hours Nurse Practitioner (NP) works primarily in an outreach capacity to provide advanced clinical assessment and consultation of those clients who are experiencing homelessness. The NP will initiate referral pathways that direct clients toward mainstream primary health, psychosocial and housing services. Working in collaboration with an Aboriginal Health Worker and Peer Support Worker, the NP will develop short-term primary health care management plans for those clients that access the service. The NP will collaborate with local GP’s and other primary health providers to deliver care in the after hours space. The NP will undertake health education and health promotion initiatives that seek to promote the health literacy and therefore health outcomes of those clients that access the service. The NP will develop and maintain collaborative relationships with internal and external stakeholders.This position requires a Working with Children Check (WWCC) issued by the Office of the Children's Guardian.
